The Frenzy Barbarian (Frenzy Barb) is a dual-wielding melee build that gains increasing attack speed and damage as combat continues through the Frenzy skill's stacking mechanic. Frenzy provides escalating attack speed bonuses while dual weapon masteries maximize damage output from both weapons. This aggressive build excels at sustained combat and boss killing, offering a frenzied berserker playstyle with incredible attack speed potential and devastating dual-weapon damage.

First thing to do is allocate 1 point in the following Skills: Bash, Sword Mastery, Axe Mastery, Mace Mastery, Polearm Mastery, Throwing Mastery, Spear Mastery, Howl, Taunt, Shout, Battle Cry, Battle Orders, Grim Ward, War Cry, Battle Command, Find Potion, Find Item, Leap, Leap Attack, Concentrate, Berserk, Whirlwind, Iron Skin, Natural Resistance, Increased Stamina, and Double Swing. After this max out the following Skills: Frenzy, Weapon Mastery (matching your main-hand weapon), Second Weapon Mastery (matching your off-hand weapon), Battle Orders, Shout. With your extra points, max out Natural Resistance for survivability and then Iron Skin for additional defense and life.

When it comes to your Attributes, start with acquiring a Hellfire Torch and an Annihilus Charm. Put those in your inventory and then add Strength, enough to wear your gear and meet both weapon requirements. Dexterity should meet weapon requirements and can be invested for Attack Rating if needed. Energy is not necessary with Battle Orders and proper gear, the rest of your points should go into Vitality for maximum survivability during extended melee combat.

Best Weapon choices include Grief Runeword in Phase Blades for both hands due to massive damage, Ignore Target Defense, and no repair costs, or Death Runeword in one hand with Grief in the other for Deadly Strike synergy. Oath Runeword is an excellent budget option in Ettin Axes or War Swords. Last Wish paired with Grief offers incredible sustainability with Life Tap proc. Since Frenzy requires dual-wielding, no shield can be used. When it comes to your Armor, Fortitude Runeword in a Sacred Armor base is Best in Slot for the 300% Enhanced Damage that affects both weapons. Duress Runeword is a budget alternative with Enhanced Damage, Crushing Blow, and Open Wounds. Treachery Runeword provides 45% Attack Speed and Fade proc for survivability. Helmet, Arreat's Face is ideal for +2 Barbarian Skills, Life Leech, Damage Reduction, and 30% Faster Hit Recovery, or Guillaume's Face for Crushing Blow and Deadly Strike. For Gloves, Laying of Hands provides 20% Attack Speed and 350% Damage to Demons, or Dracul's Grasp for Life Tap proc, Life Leech, and Strength bonus. For the Belt, String of Ears is excellent for 15% Damage Reduction and 8% Life Leech, or Verdungo's Hearty Cord for 40 Vitality, 15% Damage Reduction, and 10% Faster Hit Recovery. When it comes to Boots, Gore Rider for 15% Crushing Blow, 15% Deadly Strike, and 10% Open Wounds, or War Traveler for Magic Find and 25-35 Strength/Vitality. Rings, Raven Frost is essential for Cannot be Frozen, 15-20 Dexterity, and 150-250 Attack Rating, pair with Bul-Kathos' Wedding Band for +1 Skills and 5% Life Leech. Amulet you can opt for Highlord's Wrath for 35% Deadly Strike and 20% Attack Speed, Mara's Kaleidoscope for +2 Skills and resistances, or a rare amulet with +2 Barbarian Skills, Life Leech, Attack Rating, and resistances.

For your inventory, focus on Barbarian Skill Charms with +Combat Skills and life bonuses. Small Charms with 3% Max Damage/20 Attack Rating/20 Life combinations are excellent for sustained combat. Attack Rating Small Charms ensure consistent hits with both weapons. Life and Resistance Small Charms provide survivability for melee combat. Damage Reduction Small Charms stack well with other sources. Gheeds Fortune for Magic Find and reduced vendor prices.

Mercenary wise, an Act 2 Nightmare Offensive mercenary with Might Aura amplifies your dual-weapon damage output significantly. For Weapon, Infinity Runeword in a Giant Thresher base breaks Physical Immunities and provides Conviction Aura for Attack Rating, or Pride Runeword for Concentration Aura damage bonus. Helmet options include Vampire Gaze for Damage Reduction and Life Leech, or Andariel's Visage for Attack Speed and Strength bonus. Armor you want Fortitude Runeword in an Ethereal Sacred Armor base for maximum damage support to complement your dual-wielding, or Treachery Runeword for Attack Speed and Fade proc.
